Dementia Link Workers

Alzheimer’s Australia SA’s Dementia Link Workers are a specialised team which assists people with dementia and their families to access specific information, resources, education and support, tailored to their individual needs. The Dementia Link Worker program is funded by the Commonwealth Home Support Program (CHSP).

What do Dementia Link Workers do?

The Dementia Link Worker program is a service to assist people 65 years or over or Aboriginal and Torres Straight Islander (ATSI) clients 50 years or over living in the community with a diagnosis of dementia, and their families. Dementia Link Workers now cover the whole state and provide one-off assistance or work more intensively with people living with dementia and their families if further support is required. Dementia Link Workers will also assist special needs groups such as the CALD and ATSI community members 50 years and over, people at risk of homelessness 50 years and over, veterans, members of the LGBTI community, and people living alone.
